Women's Health Nurse Practitioners (WHNPs), Certified Nurse Midwives (CNMs), and Family Nurse Practitioners (FNPs) provide a substantial and growing portion of US ambulatory women's health. Rural counties, community health centers, Title X clinics, Federally Qualified Health Centers, tribal health programs, and telehealth women's health practices — the exact settings where prolapse, postpartum SUI, and pelvic-floor complaints most often present. Yet the traditional Milex + Bioteque distributor channels route through OB-GYN group buying accounts, leaving many NP-led clinics without a clean supply relationship. SciMed sells direct to any licensed clinician or practice — no OB-GYN affiliation required.

A note on scope of practice

Important: Pessary fitting and management sit within recognized advanced-practice scope for NPs and CNMs in many US states, particularly those with full or reduced practice authority for APRNs. Rules vary significantly by state and by practice setting — including whether independent, collaborative-agreement, or supervisory language applies. SciMed does not provide legal guidance on your scope. Confirm current requirements with your state Board of Nursing and your malpractice carrier before initiating pessary services.

That said, pessary fitting by NPs and CNMs is well-established in states that recognize full or reduced APRN practice authority. Practices commonly cited by our NP customer base as places where NP-fitted pessary is routine include: California, Colorado, Oregon, Washington, New Mexico, Arizona, New York, Massachusetts, Vermont, Maine, Minnesota, Iowa, Montana, Alaska, Hawaii, Nevada. This list reflects patterns reported by our customers and is not legal advice.

Telehealth-compatible fitting workflow

Yes — pessary care fits inside a telehealth workflow. The 2024 TOPSY RCT (NCBI NBK603994) demonstrated non-inferiority when patients were supported in structured self-management, including self-removal and reinsertion cycles guided by clinicians. A separate 2024 RCT (Springer, DOI 10.1007/s00192-024-05933-x) confirms silicone users self-manage more successfully than PVC.

Practical workflow for NP + CNM telehealth practices: (1) in-person or hybrid initial visit for symptom assessment + initial sizing using the SciMed starter kit; (2) patient training on self-insertion + hygiene; (3) video follow-up at 2 weeks and 3 months; (4) ongoing supply via direct-to-patient shipping at $49.99. Patients who cannot self-manage stay on traditional in-clinic exchange schedule.

Regulatory + reimbursement

  • FDA: Class II, 510(k) Cleared, Product Code HHN, regulated under 21 CFR 884.5350
  • HCPCS A4562 — Pessary, non-rubber, any type. HSA/FSA eligible.
  • CPT 57160 — Fitting and insertion of pessary or other intravaginal support device (the clinical service you bill)
  • Medicare Part B (2026 CMS DMEPOS): A4562 allowable $69–$89 by MAC
  • HRSA-eligible community health centers can use SciMed as a direct medical-supply vendor

Peer-reviewed research to support your documentation

  • TOPSY RCT (2024) — Bugge C et al. Self-management of vaginal pessary for POP. NCBI NBK603994. Supports patient-directed management as valid model.
  • Springer 2024 RCTDOI 10.1007/s00192-024-05933-x. Silicone users more successfully self-manage than PVC.
  • AWHONN + ACNM position statements on advanced-practice nursing scope in women's health, including conservative management of pelvic floor disorders. Consult current versions on respective organization websites.
  • ACOG Practice Bulletin 214 — pessary as first-line for symptomatic POP.

Frequently asked questions

Can a nurse practitioner or CNM fit a pessary without a supervising physician?

In many states, yes — pessary fitting sits within recognized advanced-practice scope, particularly in states with full practice authority for APRNs. In other states, a collaborative practice agreement or physician oversight is required. Confirm with your state Board of Nursing.

Do you support telehealth-only NP practices?

Yes. Peer-reviewed evidence (TOPSY RCT 2024; Springer 2024 RCT) supports patient self-managed pessary care with clinician oversight. We recommend at least one in-person or hybrid visit for the initial fitting where possible.

Does an FQHC or community health center need special credentialing to buy from SciMed?

No special credentialing to buy. FQHCs and RHCs purchase through standard AP workflows. For the additional 5% community-health discount, send HRSA scope designation, 340B ID, or state FQHC/RHC certificate.

Are SciMed pessaries HRSA-eligible for supply purchasing?

Yes. FQHCs and RHCs regularly use SciMed as a direct medical-supply vendor. Itemized invoices list HCPCS A4562 and are compatible with standard federal-grant supply-cost accounting.

How do collaborative-practice agreements affect ordering?

Ordering itself does not require a collaborative agreement — pessaries are Class II devices, not Rx drugs. The collaborative agreement, where your state requires one, governs the clinical fitting and management, not the supply purchase.

What about malpractice coverage for pessary fitting?

Most major APRN malpractice carriers include pessary fitting within standard women's health coverage. Confirm with your carrier that CPT 57160 is included on covered-procedures list.

Can patients buy at the practice's discounted rate?

No — volume tier is a practice-account benefit tied to your ordering. Patients can order directly at $49.99 and you can furnish an HSA/FSA-ready receipt.

Return / refund policy for a practice account?

Unopened, unused inventory in original packaging returnable within 30 days for restocking. Opened or fitted devices not returnable for infection-control reasons.

Do you ship internationally?

US only. Ships to all 50 states, DC, and US territories including APO/FPO for military women's health clinics.
