The right self-check habit catches 90% of pessary problems before they become clinic visits. Here's the schedule every daily-wear patient should follow.
Weekly self-check
Every 7 days (about 5 minutes)
- Remove the pessary. Wash your hands + the pessary with warm water + unscented soap.
- Inspect the device: any tears, cracks, cloudy discoloration, or rough spots? Should look like new.
- Quick body check: any unusual discharge (color, odor, quantity)? Any spotting? Any pain?
- Air-dry the pessary for 10-15 min.
- Reinsert. If it feels different (harder to insert, less comfortable), pay attention — may be a fit-change signal.
- Store weekly-removal date on your phone calendar. Consistency matters.
Monthly self-check
Every 30 days (about 15 minutes)
- Everything from the weekly check, plus:

Device fit assessment: Does the pessary still feel like it did month 1? Any of these changes = size may need to change:
- You can feel the pessary during normal activity (before you couldn't)
- You notice it slipping or shifting during bowel movements
- Prolapse symptoms are returning even with the pessary in
- Silicone condition: Hold the ring up to light. Any surface roughness, discoloration beyond the base color, or a chalky/sticky feel = time to consider replacement (usually 12-18 months).
- Skin health check: After removal, use a mirror to look at the vaginal opening. Any redness, sores, or irritation? A single day of pink irritation is fine; persistent redness needs provider attention.
- Weight + hormonal change check: Have you lost or gained 10+ lbs, gone through menopause, or started/stopped hormone therapy since your last fit? All can change pessary sizing.
- Persistent bleeding (fresh red, more than spotting)
- Foul-smelling discharge
- Severe pain during removal or wear
- Fever + pelvic pain
- Inability to remove after 24 hours of attempts
Signs it's time to reorder
- Surface wear — ring feels chalky, sticky, or discolored
- Fit change — needs a different size after weight or hormonal shift
- 12-18 months since first use — even undamaged silicone rings are recommended for replacement at this interval
- Traveling for a long trip — keep a spare device on hand

How long does a silicone ring pessary really last?
12-18 months of daily wear with proper care. Some patients get 24 months. Replace sooner if you see surface wear or discoloration.
What discharge is normal?
Small amounts of clear-to-white discharge are normal — the vagina self-lubricates and the pessary can slightly increase this. Yellow, green, gray, or foul-smelling discharge is not normal.
Can I skip the weekly check if everything feels fine?
No — some issues (silicone wear, mild tissue changes) can't be felt. Weekly hands-on inspection is the only reliable catch.
