Indications for Ring Pessary Without Support

Fitting Protocol

  1. Pre-fitting examination: Perform a pelvic exam to quantify prolapse stage (POP-Q system), assess vaginal capacity, and identify any vaginal atrophy requiring treatment before fitting

  3. Trial insertion: Fold the ring and insert posteriorly; rotate horizontally and advance behind the pubic symphysis
  4. Retention check: Ask patient to Valsalva (bear down) — a correctly sized pessary stays in place; if it expels, try the next size up
  5. Comfort verification: Patient should not feel the pessary during standing, walking, or sitting. If discomfort is present, check for improper positioning or consider a smaller size
  6. Voiding check: Have patient void to confirm no urinary retention

Patient Self-Management Instructions

  • Remove the pessary weekly or every 2 weeks for cleaning with mild soap and warm water
  • Reinsert after cleaning (or leave out overnight if desired)
