Clinical indications (ACOG-aligned)

  • Stage I–II uterine or vaginal vault prolapse without coexisting cystocele
  • Patients seeking non-surgical management of prolapse
  • Patients who cannot or prefer not to undergo surgical repair
  • Bridging therapy pre- or post-surgical repair
  • Self-managed daily pessary therapy

ACOG Practice Bulletin No. 214 (Nov 2019) recommends pessary therapy as first-line non-surgical treatment for symptomatic pelvic organ prolapse.

Why the open-ring design

The open-ring (without-support) configuration is the standard first-line ring pessary for patients without anterior compartment prolapse. The simple ring design folds easily for self-insertion and removal, is generally well tolerated for long-term daily wear, and can typically be worn during intercourse without removal — unlike the with-support membrane design. Read the complete open-ring guide.

Care and cleaning

Wash with warm water and mild, unscented soap before first use and between insertions. Do not use alcohol, petroleum-based products, or harsh disinfectants. The device may be steam autoclaved for clinical re-use per the included IFU. With proper care, expected device life is 1–2 years of regular use.

Sizing note: First-time pessary fitting should be performed by a qualified gynecologist or urogynecologist. If your provider has recommended a specific size, order that size. Between sizes? Order the combo pack (2 sizes) or right-size pack (3 sizes) at bundle pricing. See our complete pessary size guide for clinical fitting reference.

Frequently asked questions

Do I need a prescription to order a pessary?

A pessary is an FDA Class II medical device. Many US states permit over-the-counter purchase for personal patient use, but we strongly recommend that initial sizing and fitting be performed by a qualified gynecologist or urogynecologist. Read our full guide.

What's the difference between Without Support and With Support?

The without-support (open ring) is the standard first-line ring pessary for prolapse alone. The with-support version adds an integrated membrane across the ring for additional bladder neck support — preferred for patients with concurrent stress urinary incontinence or cystocele. Most patients start with the without-support design.

Can I wear the pessary during sex?

Yes, for most patients the open-ring (without-support) design can be worn during intercourse without removal — the open center allows penetration. This is one reason the open-ring is preferred over the with-support design for sexually active patients without cystocele. See our daily life guide.

How do I know which size to order?

Pessary fitting is typically performed by a clinician using a fitting kit. If your provider has recommended a specific size, order that size. Most US patients fit sizes 3–5 (64–76 mm). If you're between sizes or doing a re-fitting, order the 2-size combo pack or 3-size right-size pack for trial pricing.

Why are SciMed pessaries available in sizes 0–9 when most suppliers stop at 8?

Size 9 (102 mm / 4 inches) is rarely fitted but essential for patients with significantly enlarged vaginal vault — typically post-childbirth, post-hysterectomy, or with advanced prolapse. Stocking the full range means we can serve any patient your provider has fitted, including outliers. Most other US suppliers stock sizes 0–8 only.

Is the pessary FDA-cleared or FDA-approved?

SciMed ring pessaries are FDA 510(k) cleared as Class II medical devices under FDA Product Code HHN, regulated under 21 CFR 884.5350. Class II devices undergo FDA 510(k) clearance, not the more rigorous "approval" process used for Class III devices. Both indicate the device is legally cleared for US sale and use.

How long does a pessary last?

With proper care, a medical-grade silicone ring pessary typically lasts 1–2 years of regular use. Replace sooner if you notice surface degradation, discoloration, or persistent odor that doesn't resolve with cleaning. See our pessary replacement guide.

Is the pessary HSA/FSA eligible?

Yes. Pessaries are covered under HCPCS code A4561 (Pessary, rubber, any type) and qualify as an IRS Section 213(d) medical expense. Pay with your HSA/FSA debit card at checkout, or pay out-of-pocket and submit your SciMed receipt for reimbursement. Read our complete HSA/FSA guide.

Can I insert and remove the pessary myself?

Yes — open-ring pessaries are the most commonly self-managed type. Most patients learn the insertion technique from their gynecologist during the fitting visit and can manage independently at home. See our step-by-step insertion guide.

What if the pessary doesn't fit?

If your provider says you need a different size, return the unopened pack within 30 days for a free size exchange. Same-day re-dispatch on the replacement — free shipping both ways. If you opened it and the fit isn't right, contact us at sales@scimedstore.com — we can guide you through a re-fit.

How discreet is shipping?

All orders ship in plain, unmarked outer packaging. Shipping labels show "SciMed Store" only — no product names, no medical references. Billing on your card statement reads "SciMed Store" only.
